HOW DO I BECOME A PROVIDER FOR NET?

           

   

Individuals who wish to become a provider for NET must sign an acknowledgement form that recognizes the individual is a volunteer who is providing transportation services under the Non Emergency Transportation Program. They are classified as an “Independent Contractor” not an employee of the Noble County Department of Job and Family Services.  Such individuals will not be acting on behalf of the Noble County Department of Job and Family Services and/or Noble County, Ohio.

 

As an Independent Contractor, a NET provider will be performing services for Medicaid-eligible clients by transporting them to and from medical appointments, drug stores, etc. since the clients are unable to transport themselves.  In exchange for providing this service, the provider’s  reimbursement will be mileage reimbursement in accordance with the travel-reimbursement policy established by the Noble County Department of Job and Family Services at the time the transportation services are provided.

 

Individuals who wish to become NET providers must agree to provide the Noble County Department of Job and Family Services with a copy of a valid driver’s license, current automobile insurance, and social security number prior to providing any transportation services. It is the responsibility of the provider to notify the Noble County Department of Job and Family Services immediately of any suspension of their driving privileges and/or any lapse in their automobile insurance.  The individual shall not  provide any transportation services until such time that driving privileges and/or automobile insurance has been re-instated and proof is submitted to the Noble County Department of Job and Family Services.
